Six questions, asked of every group.
Qognexus™ is the instrument behind every score in the platform. It measures readiness for a specific change, at the level of the people it actually lands on, not as one organisation-wide average that hides who is struggling.

How the score is built
One scale, applied the same way every time.
1–5
Every dimension scored from the group's own responses, not an assessor's opinion.
Below 2.5
A critical flag. It stands on its own for governance, whatever the overall score says.
Per group
Never a programme average on its own. A 3.9 programme with one group at 2.3 is a flagged programme.
The same six
Pulses re-ask the same six dimensions, so movement is measured rather than re-estimated.

The six dimensions
The Qognexus™ Framework
Each dimension is a question asked of every impacted group and scored independently. Together they cover understanding, willingness, capability, capacity, structure and leadership.

Do they understand the change?
Whether the group can accurately describe what is changing and why. It is the precondition every other dimension depends on.
Intent

Do they want it?
Willingness to engage, separate from understanding. A group can grasp a change completely and still resist it.
Grounding

Can they actually do it?
Skills and practical capability to perform the new way of working, distinct from wanting to.
Need
NED
Do they have capacity for it?
Bandwidth to absorb this change alongside everything else already competing for the group's attention.
Anchoring

Is the structure behind them?
Whether incentives, processes and performance systems sustain the new way of working rather than quietly rewarding the old one.
Leadership

Is leadership actually leading it?
Quality and reach of sponsorship, from executive commitment through to frontline manager activation.
Why it is held per group
An average hides the group about to fail.
A single organisation-wide score can read as healthy while one function is in real trouble. Qognexus™ never collapses groups into one number. Every score, flag and trend line stays attached to the group it describes.

From score to action
A low score isn't the end of the analysis.
Each dimension below threshold routes to a different kind of response, because a group that does not understand the change needs something other than a group that understands it and is actively resisting. Five of the six map to a named resistance type; the sixth is the sponsor's to answer.

Leadership is deliberately not a resistance type. A leadership gap is not the group's resistance to overcome. It names the sponsor responsible for that group and the visible action expected of them.
